Enhancing the safety of transitions in care in rehabilitation and complex continuing care.

Transitions in care challenge safety and autonomy at all stages of the healthcare continuum; however, the transition from rehabilitation and CCC to home is particularly critical and not well understood.

This presentation highlights the challenges and proposed solutions for a collaborative, interprofessional process that involves patient experiences as essential resources to ensure a transition to home that is safe.

While safety environments across acute care and rehabilitation settings differ in terms of length of stay, urgency of safety issues and complexity of human factors, there are common elements that are essential for a safe and reliable healthcare setting.
