The following statement discloses Toronto Rehab's information gathering and dissemination practices for www.torontorehab.com. We collect your IP address for statistical purposes only. The web site does not use cookies.
Any information collected from registration or order forms, including credit card numbers and expiry dates, is transmitted through a secure server and is used only for the purpose for which it was sent. Any e-mail addresses sent to us are kept confidential and are used only to reply or send information if authorized by the e-mail account holder. Information collected through this web site is not shared with or sold to any third-party.

Protecting your personal and health records when you are a patient or former patient
As a Toronto Rehab patient, the information you share with your health care team is considered personal to you. We respect your privacy and your right to have your personal and health records kept confidential. To this end, Toronto Rehab also requires staff, volunteers, students and physicians to sign a confidentiality agreement.
Hospital practices are in place to ensure your information is secure and that you are advised should we need to share it. Only Toronto Rehab staff involved in your care and select employees of the Ministry of Health and Long-Term Care may access your file for health related purposes, billing and statistical reporting.
Toronto Rehab values your privacy and agrees not to ask for more information than is required by law or needed to provide you with the best care. Your personal information may be used to:
- Provide the best patient care
- Improve the quality of hospital services
- Help researchers answer questions about illness and disease
- Fulfill provincial regulatory requirements
- Provide general information about the hospital and occasionally ask for support of fundraising initiatives
Your request for health care services implies consent for the handling of your personal information for these purposes. It is your right to withdraw consent at any time. If you have any questions or do not want your information used for a specified purpose, please let us know. Your request must be in writing and may be subject to various obligations and reasonable notice. Please contact the Chief Privacy Officer. For more information, talk to Health Records staff at the Toronto Rehab centre where you receive your care.
The privacy practices in place at Toronto Rehab are consistent with similar practices in place at hospitals across Canada and are based on the federal legislation Personal Information Protection and Electronic Documents Act and the provincial legislation Privacy of Health Information Protection Act.
