Detailed admission criteria for Toronto Rehab’s Spinal Cord Rehab Program and all other programs and services, are available in our Guidebook for Referring Professionals.
- To refer a patient for inpatient services, fax a completed GTA Rehab Network Inpatient Referral Form and all medical and surgery consult notes and therapy notes to the Inpatient Service Coordinator at 416-422-2467 or call 416-597-3422, ext. 6105.
- To refer a patient for outpatient services, fax a completed Outpatient Service Referral Form to the Outpatient Service Coordinator 416-597-7042 or call 416-597-3422, ext. 6188 or 6151. If the referral is for the Fitness Centre ONLY, please use the Fitness Centre Referral Form. If the referral is for multiple services, including the Fitness centre, please use BOTH the Outpatient Service Referral Form and the Fitness Centre Referral Form.
