• In addition to individualized treatment plans, every patient in Toronto Rehab’s Spinal Cord Rehab Program has a personal goal coordinator who actively ensures that the patient’s needs and goals are met in a culturally and linguistically appropriate way.
• Patients’ family and friends are actively involved in designing and implementing the care plan. Through meetings, written updates, education sessions and one-on-one interaction, family and friends learn about the life-altering effects of spinal cord injury and work with the care team to set goals, discuss progress and plan for discharge.
• Psychology and social work provide emotional counselling and support to patients and their families.
• Toronto Rehab partners with community-based organizations to assist patients as they transition from inpatient to outpatient services and to life outside of the hospital.
