Individualized stroke rehabilitation

In addition to individualized treatment plans, every patient in Toronto Rehab’s Stroke Service has a personal goal coordinator who actively ensures that the patient’s needs and goals are met in a culturally and linguistically appropriate way.
Patients’ family and friends are actively involved in designing and implementing the care plan. Through meetings, written updates, education sessions and one-on-one interaction, family and friends learn about the life-altering effects of strokes and work with the care team to set goals, discuss progress and plan for discharge.
Emotional counselling is also available to patients and their families.
Support is in place to arrange travel and accommodation for families of inpatients from outside the Greater Toronto Area.
Follow-up can be conducted using videoconferencing technology to limit patients’ travel requirements. Support is in place to arrange this type of follow-up.
