Comprehensive stroke rehabilitation
Toronto Rehab conducts detailed interprofessional assessments of stroke patients. Assessments are led by physiatrists. This is a competitive advantage in identifying deficits, individualizing rehab care plans and prognosticating.
- The Stroke Service offers a full continuum of care, from inpatient services to outpatient services to follow-up clinics. Patients can enter the service at any point along the care continuum and progress through the stages with continuity.
- Five full-time neurophysiatrists and a full-time neuropsychiatrist work within Toronto Rehab’s Neuro Rehab Program. No other neurological program in Canada has this complement of physician representation on its health care team.
- Inpatients receive daily therapy on weekdays and are taught how to continue therapy independently in the evenings and on weekends. Patients usually see three or four therapists a day. Outside of assigned therapy times, patients work closely with nursing staff to practice daily living activities. Outpatients are scheduled for therapy based on their treatment plan and availability.
- Regular involvement of physiatrists in care plan implementation enables timely reassessments to optimize the benefits of rehab for each individual patient.
- Toronto Rehab uses a primary nurse program and consistently meets provincial targets for full-time nurse percentages. This ensures patients work with nurses who have specialized skills and maximizes continuity of care and patient safety.
- In addition, the Neuro Rehab Program is one of only a few programs in the country employing rehabilitation therapists, who can follow through with any type of prescribed therapy at any time, including weekends and evenings.
