Nearly all of our patients arrive from acute care hospital after a recent stroke. In the four to five weeks we have with patients, our team must help prepare them for the road home. The effect a stroke has on a person depends on whether it occurred on the left or right side of the brain. Many patients that we see have problems that affect balance, ability to communicate, emotions, memory, and diet due to swallowing difficulties. Our team works with the individual and family members to identify the issues that are important to their quality of life. While each person's goals are different, many patients want to:

Walk again, unassisted, or with minimal aids
Perform other daily activities that require good balance and coordination
Be able to communicate, speak, and understand words
Improve one's memory

Treatment plans are regularly reviewed with patients and family members, who are encouraged to participate in order to provide the necessary support at home.

Many patients return home and continue therapy on an outpatient basis, either at Toronto Rehab or in the community, while others require the additional support provided in a long term care facility.
