• To refer a patient for inpatient brain injury rehabilitation, fax a completed GTA Rehab Network Inpatient Referral Form and Functional Information Form to 416-597-7021 or call 416-597-3422, ext. 3593 or 3441.
  
  
• To refer a patient for outpatient brain injury rehabilitation, fax a completed ABI Client Community Profile Form and all relevant consult and scan notes to 416-597-7021 or call 416-597-3422, ext. 3593 or 3441.
  
  
• To refer a patient to the Chronic Pain Service, fax a completed Outpatient Neuro Rehabilitation Referral Form to 416-597-7140.
