• To refer a patient for inpatient musculoskeletal rehabilitation, fax a completed GTA Rehab Network Inpatient Referral Form and Functional Information form to 416-537-3752 or call 416-597-3422, ext. 4506.
  

• To refer a patient for outpatient rehabilitation, call 416-597-3422, ext. 4514. To refer an outpatient to the Physiatry Clinic specifically, fax a completed Outpatient Clinic Request for Consultation Form to 416-516-6373 or call 416-597-3422, ext. 4541.
