Lakeside Long-Term Care Centre is a brand new facility providing care for individuals who are no longer able to live independently and require 24-hour nursing care.
The 128-bed facility overlooks an outdoor park and gardens in the Parkdale neighbourhood, beside Toronto Rehab's Queen Elizabeth Centre.
The facility is owned by Toronto Rehab, which has contracted with Extendicare (Canada) Inc. to operate, manage and employ the staff in the facility.

80 private rooms; 48 standard rooms (shared bathroom) with enhanced privacy design. Facility designed with four Resident Home Areas (RHA), each containing resident rooms, dining room, lounge and activity space for 32 residents. |

Variety of therapeutic programs help residents to maintain mental and physical well-being. Recreational programs provide opportunities for residents to stay active and socialize. |

In general, to qualify for admission, a person must:

Be 18 years of age or older. |
| |
Possess a valid Ontario Health Card. |
| |
Have health care needs that cannot be met with any combination of caregiving or community-based services in the home. |
| |
Have health care needs that can be met in a long-term care facility. |
