Anyone who is approved for long-term care by the Toronto Community Care Access Centre (CCAC) is eligible to be a resident at Lakeside Long-Term Care Centre. Please contact the Toronto Community Care Access Centre (CCAC) at 416-506-9888 for more information on how to apply.
