Toronto Rehab's Geriatric Rehabilitation Program provides inpatient, outpatient and community-based care to Toronto seniors who require help coping with the complications of age. Many patients are admitted directly from home, long-term care facilities, or after a stay in an acute care hospital. We provide our services in partnership with the Regional Geriatric Program of Toronto, University Health Network - Toronto General Hospital, and the University of Toronto.

Admissions Criteria for all Geriatric Services is available from our Guidebook for Referring Professionals.

Generally, a patient must:

Be over the age of 65
Have multiple medical, functional problems
Have defined rehabilitation goals
Have a desire to participate in a rehabilitation program
Have potential to benefit from therapy and improve in the areas of daily living, mobility, social functioning, communication and cognition

Please note: Not all of our services require a physician's referral.
