By referring to Toronto Rehab's Complex Continuing Care program, your patients will benefit from a wide range of therapeutic activities as well as from comprehensive inter-professional care.  Our Complex Medical Services include general complex continuing care, dialysis, palliative care, respite care and special care. We also offer one of the few Low Tolerance Long Duration (LTLD) rehabilitation programs in the GTA.

Our program, situated in a quiet residential neighborhood, offers patients beautiful grounds, easy access to nearby parks and shopping, and convenient TTC community bus service directly from the front door.

We offer patients:

  • 24-7 care provision by registered nurses and registered practical nurses, and access to inter-professional team members;
  • Access to medical care through attending physicians;
  • Onsite access to medical specialists;
  • Semi-private and private rooms for patients with additional coverage;
  • Evening and weekend activities;
  • On-site services for patients requiring augmentative and alternative communication devices, chiropody and dentistry;
  • Linkages to services and specialists in Toronto Rehab's Spinal Cord Rehabilitation and Neuro Rehabilitation programs.

Patients and their families are considered important members of the inter-professional care team.  Patients are involved in directing their own care, and team members work closely with them to identify and help achieve their goals.  A discharge plan to home or another appropriate discharge destination will be developed for all patients upon admission to Complex Continuing Care. Patients are considered ready for discharge when they consistently no longer need the level or scope of care and services provided in Complex Continuing Care.
