Patients in Complex Continuing Care require complex medical and nursing care and access to diagnostic services. Many of our patients are brain injured as the result of an accident or disease. Others have had a debilitating stroke, spinal cord injury, or are coping with the effects of degenerative disorders such as Parkinson's disease and multiple sclerosis. A range of therapies may be offered to help patients develop their abilities for the best possible quality of life. We also offer on site access to clinical and medical specialists.
Once admitted to the Complex Continuing Care program in Toronto Rehab's E.W. Bickle Centre for Complex Continuing Care, both you and your family will become vital members of your health care team. Working together, you will identify goals to be achieved while in the program, develop a care plan, and determine an approximate date for discharge home or to another appropriate discharge destination in the community. Throughout your stay with us, the team will help you to learn to manage your own care.
Depending upon the nature of your disease, illness or injury, your team will consist of health care professionals who specialize in complex medical services, palliative care or low tolerance long duration rehabilitation.
Members of your Complex Continuing Care team may include a physician, nurses, a dietitian, occupational therapist, occupational therapy assistant, physiotherapist, physiotherapy assistant, speech language pathologist, social worker, pharmacist and/or chaplain. Referrals can be made to other clinical specialists such as audiologists, dentists and other medical consultants.
Events and regular activities for patients are organized by our wellness partners. Volunteers also play a valuable role in the program visiting with patients, going on outings and hosting activities at the centre.
We encourage and welcome feedback from patients and families. While your primary point of contact in Complex Continuing Care will be your Primary Nurse and/or Associate Nurse, you are welcome to speak with the manager on your unit and with our patient relations representative at any time.
The Patient Family Council, jointly chaired by a patient and family member, assists and advises program leadership on matters relating to patient care and services to promote the highest quality of care for patients and families. All Complex Continuing Care program patients and their family members are welcome to attend and participate in the monthly meetings.
