Rehabilitation Program
Toronto Rehab's
Neuro Rehabilitation Program provides specialized rehabilitation
for adults who have impairments as a result of acquired brain injury
(ABI), stroke or other neurological condition. The program is divided
into four services: Brain Injury-Neuro Cognitive, Brain Injury-Neuro
Physical, Stroke and Neurology. Patients are admitted to one of
these services based on their functional needs at the time of referral.
The Neuro Rehab Program offers services at the University Centre
(UC) and the Rumsey Centre (RC).

The Brain Injury-Neuro
Cognitive Service (BI-NC Service) is for individuals who have acquired
non-degenerative disorders and whose primary issues are cognitive
and/or behavioural, and whose physical and medical needs tend to
be minimal. Conditions include traumatic brain injury, brain tumour,
hemorrhage, anoxic brain injury and cerebral vascular events. Patients
typically require minimal traditional medical and nursing care,
but are in need of regular cueing, orientation and other consistent
cognitive techniques. Patients may present with behavioural dyscontrol,
impulsivity and lack of self-awareness. The BI-NC Service offers
a full range of therapy provided by an interprofessional team in
both the inpatient and outpatient settings.

service provides a full continuum of care including inpatient
services, outpatient services and follow-up clinics. Patients
can enter the system at any point, or move through the system
from beginning to end. |

The
inpatient service provides a secure environment to safeguard
individuals who may be at risk of wandering. |

|
Five
beds are specifically designated for patients with a brain injury
who are slower to recover. |
|
|
Family
conferences are held during the first few weeks of service,
and throughout the patient's rehabilitation, so that the patient
and family can work with the team to set goals, discuss progress,
and plan for discharge. |

|
Toronto
Rehab is a member of the Toronto ABI Network, an organization
that coordinates admissions and service delivery among all ABI
service providers. |
|
|
Education
is focused on health promotion and secondary prevention. |
Inpatient
Service
The BI-NC Service has 16 beds on a secure 31-bed unit (this unit
includes 15 BI-Neuro Physical beds). Patients are often transferred
to BI-NC outpatient services after their inpatient stay.

Outpatient
Services
The BI-NC Service offers team-based, outpatient services at both
the University and Rumsey centres. Typical goals for patients within
this service include a return to independent community living and
return to work. The service helps brain-injured patients and their
caregivers to make the transition from inpatient care to community
living.
Chronic Pain
Service
Rumsey Centre also provides a Chronic Pain Service that offers group-based therapy, utilizing a cognitive-behavioural approach, to treat patients with chronic pain, including fibromyalgia.

The Brain Injury-Neuro
Physical Service (BI-NP Service) is for individuals who have acquired,
non-progressive neurological disorders and whose primary rehabilitative
needs focus on physical functional recovery. Most individuals will
have signs and symptoms of bilateral or diffuse brain involvement.
Patients may also have cognitive and/or behavioural deficits and
complex medical needs. Conditions include traumatic brain injury,
brain tumour, hemorrhage, anoxic brain injury and cerebral vascular
events. Patients with a diagnosis of multiple sclerosis who are
exhibiting symptoms of cerebral involvement may be referred. The
BI-NP Service offers a full range of inpatient and outpatient therapy
that is provided by an interprofessional team.

caregivers to make the transition from inpatient care to community
living. Group-based health promotion and fitness programs, in addition
to regular outpatient therapy, are provided to patients with multiple
sclerosis.

The Stroke Service
is for adults with an established diagnosis of a recent thrombolic,
embolic or hemorrhagic stroke. Patients typically have symptoms
of unilateral cerebral involvement, which could include difficulties
in such areas as: mobility, balance, coordination, speech &
language, swallowing, mild cognitive deficits, sensory/perceptual
skills, and emotional functioning. The Stroke Service offers a full
range of therapy provided by an interprofessional team in both inpatient
and outpatient settings.

Service
The Stroke Service offers team-based, outpatient services at both
the UC and RC sites. Typical goals for patients within this service
include achieving independence in activities of daily living and
returning to independent community living. The Service helps individuals
who have suffered a stroke, and their caregivers, to make the transition
from inpatient care to community living.

Acquired
brain injury |
The Neurology
Service provides comprehensive interprofessional assessment, clinical
management, and rehabilitation services to individuals who have,
or are suspected of having, a brain injury or impairments to the
nervous system. Service is provided to third-party funded clients
such as the Workplace Safety and Insurance Board, private insurance
companies, rehabilitation agencies and legal firms. Clients are
admitted from across Canada, and are independent in activities of
daily living. Depending on the complexity of the case, clients might
see a number of different clinical professionals including, physiatrist,
neurologist, physiotherapist, occupational therapist, and psychologist
over a one to two week period. If required, additional neuro-diagnostic
tests will be arranged including: magnetic resonance imaging (MRI),
CT scanning, EMG, EEG, and ENT evaluations. Testing is arranged
at Toronto General Hospital, the University Health Network.

Individuals whose accident sequelae include symptoms related
to traumatic brain injury, requiring highly specialized assessment,
clinical and vocational management and follow-up treatment |

Individuals
with mild to moderate head injuries whose symptoms reflect emotional
problems of a post-traumatic nature and who would benefit from
behavioural management treatment approaches |
|
|
Individuals
with serious neurological conditions (e.g. multiple sclerosis)
resulting in significant dysfunction |

Individuals
in whom the diagnosis is uncertain, but where the diagnosis
of brain injury might require clarification or confirmation |

This clinic
provides ongoing follow-up for all Neuro Rehab patients who have
been discharged from the Stroke, BI-NC or BI-NP inpatient services
and who are not necessarily receiving outpatient services. Patients
may also be referred directly from the community. The clinic focuses
on assessment and recommendations related to the rehabilitation
needs of patients in the community.
