Prevention and Rehabilitation of Osteoporosis (PRO) Service

Complex MSK Service

Type of service: Inpatient, outpatient
Average length of stay: 3 to 4 weeks
Conditions: Arthritis, osteoporosis, post-op complications, Parkinson's disease, diabetes, neurological dysfunctions, and cardiac conditions
Number of beds: 50
Referral required: Physician

The Complex MSK Service specializes in treating patients who have significant functional mobility and movement impairments related to the musculoskeletal system. Patients receive physiotherapy, occupational therapy, hydro/pool therapy and education to improve their level of independence in daily living.

Admitting Criteria:

Anticipated length of stay of 28 to 31 days
Medically stable
Identified achievable rehabilitation goals
Motivated and physically capable of participating in a rehab program
Cognitively able to learn new activities and follow direction
Requires interprofessional care that is not available in the community
Identified an appropriate discharge destination and an alternative plan (if required) prior to admission

Multiple Trauma Service

Average length of stay: 4 weeks
Number of beds: 4
Referral required: Physician

This new service provides rehabilitation to people who have sustained multiple fractures and other injuries e.g. in a motor vehicle accident. This service works with regional trauma centres to reduce the length of time patients currently have to wait for rehabilitation. The patient plays an integral part in their goal planning and rehabilitation.

Admissions Criteria:

An anticipated length of stay of 30 to 35 days
Two or more active orthopedic medical conditions
Multiple injuries/fractures as a result of trauma
Physically ready to participate in the entire rehab process
Has cognitive ability to learn new activities and follow direction
Over 18 years of age
Does not require active medical treatment from an acute care hospital, excluding I.V. therapy

Oncology Service

Average length of stay: 4 weeks
Number of beds: 4
Referral required: Physician

Oncology Rehabilitation is for people who have cancer of the musculoskeletal system (i.e. bone and tissue tumours, bone marrow transplants). Rehab is particularly important to minimize the potential for long-term disability among cancer survivors. Therapy focuses on helping patients regain previous levels of functioning or to adjust to new limitations. There is also an education-based outpatient component to this service.

Admission Criteria:

An anticipated length of stay of 28 to 31 days
Medically stable
Physically ready to participate in a rehab program
Cognitively able to learn new activities and follow direction
Requires limited transfers for external treatment and follow-up
Potential to achieve desired rehab goals
Has an appropriate discharge destination identified prior to admission
Requires interprofessional care that is not available in the community

Prevention and Rehabilitation of Osteoporosis (PRO) Service

Type of service: Outpatient

The PRO Service offers exercise, education and peer support to individuals with osteoporosis or to those who are at risk of developing the disease.

Patients learn about:

Osteoporosis medications and how to manage the disease
Appropriate exercises for people with osteoporosis
Knowledgeable and healthy food choices
Lifestyle changes
How to cope with pain and emotional issues

The service team includes a physiatrist, physiotherapist, and exercise specialist. Patients will also have consultations with a pharmacist, nutritionist, and occupational therapist. All patients receive a medical assessment, that includes bone density tests and exercise testing to determine fitness levels, in order to establish the diagnosis and severity of osteoporosis. Patients receive ongoing support and regular medical reassessments, group exercise classes, and physiotherapy intervention.
