Lakeside Long-Term Care Centre is a brand new facility (opening May 2004) providing care for individuals (mostly seniors) who are no longer able to live independently and require 24-hour nursing care. The facility is located next door to Toronto Rehab's E.W. Bickle Centre for Complex Continuing Care, home of the Complex Continuing Care Program.

The facility is owned by Toronto Rehab, which has contracted with Extendicare (Canada) Inc. to operate, manage and employ the staff in the facility.

Admission Criteria

Admissions and waiting lists are managed by the Toronto Community Care Access Centre, 416-506-9888.

To qualify for admission, a person must:

Be 18 years of age or older
Possess a valid Ontario Health Cardy
Have health care needs that cannot be met with any combination of caregiving or community-based services in the home
Have health care needs that can be met in a long-term care facility

Description of Facilities

Four Resident Home Areas (RHA) containing resident rooms, dining room, lounge and activity space for 32 residents.
Landscaped courtyard and outdoor terraces on upper level provide secure outdoor space.
Rooms accommodate pictures, bedspreads and small pieces of furniture from home. Each room has a secure, lit memory box outside the door for displaying personal items.
Dining rooms overlooking courtyard; country kitchen in each RHA provides space for informal dining and home baking; private dining room available for hosting family events.
Interfaith spiritual room.
Beauty salon and barbershop on site.
Wandering resident safety system throughout facility. Call bells in all rooms.
Public gardens and park beside the Centre provide additional outdoor space.
Easy access from the Gardiner Expressway and King Street streetcar. On-site paid parking.

Service Highlights

Physiotherapy and other therapeutic programs help residents to maintain mental and physical well-being.
Recreational programs provide opportunities to stay active and socialize.
