Toronto Rehab's
Geriatric Rehabilitation Program offers inpatient and outpatient
rehabilitation services that enhance independence and quality of
life for older adults with complex medical, cognitive and psychosocial
needs. The program is part of the Regional
Geriatric Program of Toronto - Central Service (RGP).

Geriatric Rehabilitation
benefits elderly people who are experiencing multiple physical,
emotional, cognitive and social conditions that interfere with their
independence and quality of life. Treatment is designed to improve
personal functioning to the highest possible level so patients can
return home to live in their community.

Dialysis
Service (a new service)
Number of beds:

This new service
provides patients with daily hemodialysis and daily rehabilitation.
The dialysis service is located at the University Centre and staffed
by University Health Network dialysis professionals. Dialysis patients
receive two hours of dialysis six days a week. The shorter, more
frequent dialysis means patients have more energy and are better
able to participate fully in daily rehabilitation. Patients must
arrange to have a permanent dialysis spot at a community hospital
on discharge.
On admission,
an interprofessional team of specialists assesses patients. Individualized
treatment plans are then developed for each patient based on personal
goals and abilities.

On admission,
patients are assessed by an interprofessional team. From this, individualized
treatment plans are developed for each patient based on personal
goals and abilities. Therapy is usually provided in individual sessions
and is designed to restore physical function, and promote socialization
and reintegration of the patient in the community. Patients generally
attend therapy a couple of half-days a week for up to three to four
months.

Geriatric Psychiatry
specializes in assessing and treating elderly persons who have behavioural
challenges related to Alzheimer disease and other types of dementia.
Care is provided in a secure environment.
Treatment focuses
on issues such as dementia-related agitation, wandering, aggression
and resistiveness to care. The goal is to maximize the patient's
level of functioning and to minimize the burden of caring for (non-professional)
caregivers.
