Geriatric Rehabilitation Program

Toronto Rehab's Geriatric Rehabilitation Program offers inpatient and outpatient rehabilitation services that enhance independence and quality of life for older adults with complex medical, cognitive and psychosocial needs. The program is part of the Regional Geriatric Program of Toronto - Central Service (RGP).

Geriatric Rehabilitation Service

Geriatric Rehabilitation benefits elderly people who are experiencing multiple physical, emotional, cognitive and social conditions that interfere with their independence and quality of life. Treatment is designed to improve personal functioning to the highest possible level so patients can return home to live in their community.


Dialysis Service (a new service)

Number of beds: 12

This new service provides patients with daily hemodialysis and daily rehabilitation. The dialysis service is located at the University Centre and staffed by University Health Network dialysis professionals. Dialysis patients receive two hours of dialysis six days a week. The shorter, more frequent dialysis means patients have more energy and are better able to participate fully in daily rehabilitation. Patients must arrange to have a permanent dialysis spot at a community hospital on discharge.

On admission, an interprofessional team of specialists assesses patients. Individualized treatment plans are then developed for each patient based on personal goals and abilities.

Admission Criteria:

Over the age of 65
Multiple medical, functional, emotional and/or social problems
Defined rehabilitation goals
Desire and ability to actively participate in a rehabilitation program
Potential to improve in the areas of physical functioning, activities of daily living, mobility, social functioning, communication and cognition

Type of service: Outpatient
Referral required: Personal, family and physician referrals are accepted

On admission, patients are assessed by an interprofessional team. From this, individualized treatment plans are developed for each patient based on personal goals and abilities. Therapy is usually provided in individual sessions and is designed to restore physical function, and promote socialization and reintegration of the patient in the community. Patients generally attend therapy a couple of half-days a week for up to three to four months.

Admission Criteria:

Over the age of 65
Experience some difficulty with activities of daily living, mobility, cognition, social functioning and/or communication
Have the ability and desire to attend and participate in a rehabilitation program
Have a family physician in the community
Are capable of accessing transportation to and from the Geriatric Day Hospital
Do not require one-to-one supervision to ensure safety while attending
Can benefit from two or more professional services including: Medicine, Nursing, Occupational Therapy, Pharmacy, Physiotherapy, Social Work, Speech-Language Pathology, Therapeutic Recreation, and Dietetics

This clinic conducts specialized geriatric medical assessments of seniors who are experiencing:

Deterioration of memory or cognition
Multiple physical disabilities
Mobility problems, falls or decreased physical function
Bowel or bladder difficulties
Swallowing or communication problems
Challenges coping with multiple chronic illnesses or multiple medications
Inadequate family or community supports
Possible abuse or neglect, alcoholism, poor nutrition or weight loss

Following the assessment, recommendations and referrals may be made to other hospital, outreach or community-based programs and services.


Geriatric Psychiatry Service

Conditions: Alzheimer, Lewy bodies, and related dementia
Number of beds: 20
Referral required: Health care professional

Geriatric Psychiatry specializes in assessing and treating elderly persons who have behavioural challenges related to Alzheimer disease and other types of dementia. Care is provided in a secure environment.

Treatment focuses on issues such as dementia-related agitation, wandering, aggression and resistiveness to care. The goal is to maximize the patient's level of functioning and to minimize the burden of caring for (non-professional) caregivers.

Service Highlights:

Comprehensive assessment of psychiatric, medical, functional, physical and psychosocial status of each patient.
Individual care plans address each patient's physical, cognitive and social functioning needs.
Patients are encouraged to actively participate in group activities, which focus on enhancing and maintaining functional abilities.
Upon discharge, clinical findings and recommendations about future care are shared with the family, the patient's physician and other care providers. Supportive counselling for families and caregivers is also available.

Admission Criteria:

Diagnosis of dementia
Psychiatric complications of dementia including behavioural disturbances such as agitation, aggression, depression and/or delusions
Age 65 or older (younger individuals are accepted on a case-by-case basis)
Medically stable

Type of service: Outpatient

The clinic provides specialized psychiatric assessments of elderly persons experiencing:

Depressed mood
Symptoms of late onset schizophrenia
Paranoia, delusions or hallucinations
Anxiety disorders
Behavioural disturbances associated with dementia
Other psychiatric disorders

An assessment includes a comprehensive psychiatric examination, consultation with the referring physician and other professional caregivers, and referrals to other appropriate community and hospital-based programs and services as required.

The interprofessional Outreach Team conducts assessments, on behalf of service providers in the community, and provides consultations to elderly patients and their families in the home or in a long-term care facility.

The team:

Assesses the person's support network
Evaluates accessibility factors in the care setting
Links patients and families to appropriate support services
Identifies patients who may benefit from an inpatient assessment and/or rehabilitation program
