Your Rehab Program

Each patient at Toronto Rehab has an individual rehabilitation program. 

We will work with you to develop the best rehab program for your needs. When you arrive, we will ask you about your goals for rehab. Rehabilitation goals include physical goals, as well as emotional, educational and other needs.

We will work closely with you to set goals you can achieve. Throughout your stay, we will work with you to reach your goals.

Your rehab program will be based on your overall health and needs. This is a list of some of the information we will need from you to create your care plan:

  • past illnesses, surgeries and hospital stays

  • allergies

  • any medications you are taking, including prescription, non-prescription (over the counter) and herbal medications

  • any private insurance, advance care directive, power of attorney or living will you may have, or any other information about your health care choices

  • any values or spiritual beliefs that you want us to know when planning your care

Your therapy will vary depending on the program you are enrolled in, your goals, your condition and your progress. In your spare time, your team may encourage you to do more activities on your own or with help from family or friends. You may also be involved in other group or individual activities.

The timing and intensity of your therapy will vary depending on your condition and your progress.

We want to be sure your rehab program is right for you. Your team members will be happy to answer your questions. If you are not comfortable with your treatment plan at any time, we encourage you to discuss changes or alternatives with your care team. You also have the right to refuse treatment.

When discussing your care plan with team members, here are some questions you may want to ask:

  • What are the benefits and risks of each treatment?
  • Who will provide my treatment?
  • Is my treatment part of a research study?
  • What can I reasonably expect from my treatment?
  • Are there any long-term effects my treatment might have on my quality of life?
  • What do I need to do during and after I leave the hospital? What does my family need to do?
