Your Health Record
Your health record includes:

information collected from you and your family

information from your medical report from before you came to the hospital

information about meetings held with your care team and family

details about your medications, vital signs and treatments

clinical observations

test results
You can see and read your health records at any time. Your care team can help you read the chart and explain any unknown terms.
Unless you have expressly withheld or withdrawn your consent, we will assume your implied consent to share your personal health information with other care providers within and beyond the hospital, such as your family doctor or a medical specialist, for the purpose of providing care. This is called the “circle of care” and aligns with provincial privacy legislation.
We need your written permission to let anyone other than your circle of care members, see your health record. Once we have your permission, we will forward your health record to other health care providers at no charge. If you request copies of your health record for other reasons (e.g. an insurance claim), these are available from our Health Records Department for a fee.
Family members can see a patient’s record with permission from you or from your legal representative (if you are unable to give permission). Your care team can explain how this works.
