Staff identification
It can be hard to keep track of who is caring for you. All Toronto Rehab staff and volunteers wear photo identification (ID) badges. Students also wear identification.
Look for this badge on anyone who is caring for you. If you have any concerns about the identity of the person taking care of you, or if you notice someone on your floor who is giving care but is not wearing an ID badge, please tell a member of your care team.
Patient identification
For your safety, all patients are given a wrist ID bracelet when they get here. Please wear this bracelet at all times while you are in the hospital. This bracelet:

We have a policy that requires staff members to confirm a patient’s identity in two different ways every time they need to provide care or treatment.
This means that, in addition to wearing your ID bracelet, you may be asked to:

have a photo taken for your patient record

tell us your first and last name

tell us your date of birth

show your health card number or your Toronto Rehab hospital number/medical record number on a Toronto Rehab hospital card

have a family member, substitute decision maker or two familiar staff members confirm your identity
