Checklist for Leaving

The day before you leave you should:

• Take your belongings, all of your personal items

• Settle your account with the Business Office (during business hours). Costs you might need to settle might include: semi-private or private room costs not covered by your insuranceo co-payment fees, telephone service, including long distance charges

• Return any equipment you have borrowed from the hospital (or you will be charged)

• Make sure your transportation to leave is arranged. You are responsible for arranging your own transportation when you leave the hospital.

On the day you leave, you should:

• Get dressed and finish packing (ask your care team for help if you need it).

• Eat breakfast.

• Make sure you have your equipment, medications and prescriptions and any special discharge instructions or information.

• Check out of your room by 9:00 a.m. (unless otherwise arranged with your care team).

You are responsible for arranging your own transportation home from the hospital. If a family member or friend is not able to pick you up by 9:00 a.m., ask your care team about places in the hospital to wait, unless otherwise arranged with your care team.
