Talking With Your Care Team

While you’re a patient at Toronto Rehab, you will have many opportunities to talk to your care team.  During these times, it is important that you tell your team about what is most important to you.

Examples of when you will talk to your care team about your needs are:


When you arrive

  • It is important for your care and safety that you give us up-to-date information.

  • Your care team will ask you for information that helps us to provide better care for you.

During goal setting

  • Your rehab program will be based on the goals you set for your time in rehab.

  • Soon after you arrive here, we will work with you to set goals that you can reach.

  • We will ask about your goals during your stay to make sure they are still important to you. 

  • Goal-setting is an important way to be involved in your care.

Each day, during your care

  • Every time your doctor, nurse or other member of your care team is working with you, you can ask questions or express concerns.

  • If staff are not able to address your concerns, you or your family members can contact the program service manager.

In meetings

  • You or your care team can ask for a meeting to talk about your care.

  • You may want to call a meeting if you have questions about your rehab program or specific parts of it.

At any time, you are encouraged to:

  • ask questions

  • tell us your concerns

  • ask for any information that relates to your healthcare
