Our interprofessional health care team works with individuals and their families to an individualized treatment plan that meets their needs and goals. We encourage people to take an active role in their own rehabilitation and support them with the information they need. We also help them prepare to return to living safely on their own or with assistance.

At Toronto Rehab, we also provide a full range of interprofessional care in our outpatient and follow-up clinics to help patients continue the progress they began while in hospital. We coordinate the care to ensure a smooth transition when people are discharged from hospital.

We integrate the latest research into all the care we provide. In fact, our researchers work side by side with the health care team. Our patients participate in research and benefit from discoveries—as we make them.

We’re also sharing our innovations with rehabilitation providers across the country and close to home.For example, our new model of care for hip fractures is now being rolled out across Ontario. It shortens hospital stays without impacting patient care. As a result, more people will be able to receive the rehabilitation they need and limited health care resources will go farther.We are quickly becoming a national leader in improving access by delivering rehabilitation more efficiently.
