Stroke Rehab
Stroke affects everyone differently.
At Toronto Rehab, our Stroke Rehab Program focuses on superior outcomes for each and every person.
Our interprofessional health care team works with individuals and their families to develop an individualized treatment plan that meets their needs and goals. We help individuals who have had a stroke to recover or improve:
-
cognitive skills such as memory, concentration, problem-solving
-
communication and language skills
-
balance, movement and walking
-
strength and endurance
-
emotional functioning
Education is integrated into all aspects of the care we provide to allow patients and families to fully participate in their rehabilitation and recovery. Patients also receive an individualized binder of information and educational material to help them throughout their stay and after discharge.
Individualized Therapy Promotes Recovery
Inpatients receiving care on our inpatient unit have daily rehabilitation therapy from Monday to Friday. They usually see up to three or four specialized therapists a day, depending on their needs.
Therapy is individualized and may include a combination of physical, occupational and speech therapy. Diet, exercise and lifestyle modification may be required. We also provide emotional support and counselling.
Inpatient care is provided in a 24-bed unit at University Centre. The average length of stay on the unit is five-six weeks.
After they are discharged from hospital, individuals may continue their rehabilitation at University Centre or Rumsey Centre in our outpatient clinic, day hospital or follow-up clinic.
Restoring Independence
Our goal is to help individuals regain their life and their independence.
An important part of our care focuses on helping patients to make the transition to their return home. We work with individuals and their families to make sure the necessary supports are in place at home and in the community. And we follow up with patients after discharge to help them in their transition.
Toronto Rehab education programs and support groups are also available to help individuals and families adjust to the physical and mental challenges they are experiencing.
Specialized programs for Stroke Survivors
At Toronto Rehab, we have several specialized stroke-related services:
The Augmentative and Alternative Communication (AAC) Clinic helps people who are having difficulty speaking or writing. It provides customized solutions using computers and other electronic devices.
The Risk Factor Modification and Exercise Service provides a personalized exercise and education program to help individuals reduce their chances of another stroke.
