Inpatient Geriatric Rehabilitation Service

The Geriatric Rehabilitation Service provides inpatient care for people over age 65 who need help managing health problems associated with aging.

These problems can include physical, mental, emotional or social conditions that are affecting their independence and well-being.

At Toronto Rehab, we have particular expertise in caring for older adults with complex health issues. Individuals may come to us after a stay in an acute care hospital, from a retirement or long-term care facility or from their home. Individuals usually remain at Toronto Rehab for six to eight weeks.

 

Personalized Care and Solutions

Our interprofessional team works closely with each patient and family to do a detailed health assessment and develop goals and a treatment plan. We provide rehabilitation therapy, activities and strategies to help individuals:

• manage chronic pain

• improve mobility, balance and walking

• address memory and thinking problems

• manage everyday activities independently and safely

Care is individualized. Our interprofessional health team works with individuals and their families to set meaningful goals for their needs and situation. We focus on superior outcomes for each and every patient.

 

A Focus on Safety

Falls are the number one cause of loss of independence for older adults. We work with each individual to assess the factors that might contribute to a risk of falling. There are often many factors that need attention, such as:

• poor balance or problems with gait

• vision issues

• effects of medication

• incorrect fitting or use of walkers or canes, or

• problems transferring from a wheelchair to a chair, toilet or bed

We find and implement practical solutions to reduce the risk of falls and injury. We also address other safety and wellness issues, such as strategies to help people remember to take their medications or to maintain a healthy diet.

Our goal is to help individuals return to living as safely and independently as possible in their homes – whether that be a private home or apartment, an assisted living facility, a retirement home or long-term care.
