Living Independently Longer
Cooking dinner. Going for a walk. Shopping for groceries. Visiting friends. Our days are made up of many activities that we take for granted. These activities allow us to be independent and live our lives the way we want to.
Unfortunately, illness and disability often reduce our independence as we age.
At Toronto Rehab, we help individuals to maximize life and live independently for as long as possible.
We provide rehabilitation to help people maintain or regain abilities affected by injury or age-related illness. Through research, we’re finding innovative solutions to help people live safely and independently longer.
Specialized Services
Toronto Rehab has particular expertise in caring for older individuals with multiple and complex health issues.
Our inpatient Geriatric Rehabilitation Service and Geriatric Day Hospital provide care for people over age 65 who have physical, mental, emotional or social conditions that are affecting their independence and well-being. We provide rehabilitation therapy to help individuals:
-
manage pain
-
improve mobility, balance and walking
-
address memory and thinking problems
-
manage everyday activities independently and safely
At Toronto Rehab, care is individualized. Our interprofessional health team works with individuals and their families to set meaningful goals for their needs and situaion. We focus on superior outcomes for each and every patient.
Supporting Independence
Independence can mean different things to different people, depending on their health and situation.
Some people leave Toronto Rehab and return to living on their own. Others will need the help of family or a daily caregiver. For others, home will be an assisted living facility, a retirement home or long-term care.
Our goal is to help people live safely and independently for as long as possible, regardless of where “home” is.
