Geriatric Day Hospital

The Geriatric Day Hospital is an outpatient rehabilitation program that helps older adults with physical, mental, emotional or social conditions that are affecting their independence and well-being.

The program helps individuals:

• who need additional rehabilitation services after they leave hospital or

• who are living in the community and need services to help them remain independent.

At Toronto Rehab, we have particular expertise in caring for older adults with complex health issues.

Our interprofessional team works closely with each patient and family to do a detailed health assessment and develop goals and a treatment plan. Therapy is provided on an outpatient basis twice a week for three to four hours a day. Patients usually receive care for about three months.
