All of us hope to stay healthy and live independently in our homes well into old age.

But one in two Ontarians will be touched by disability—either personally or in our immediate family.

And many of us will live with a chronic disease—such as diabetes, heart disease, osteoporosis or arthritis—as we age. In fact, approximately 90 per cent of people over age 65 have at least one chronic disease. Eighty per cent have at least two.

At Toronto Rehab, we’re experts in helping older adults with complex health issues.  

We find strategies to help people with Alzheimer’s manage their diabetes. We provide dialysis to elderly patients whose advanced kidney disease is complicated by other illnesses. We help older people who have fallen and broken their hip to walk again after weeks in bed has left them frail.

We are dedicated to helping people maximize their life. We help individuals remain active so that they can participate with family and friends in the things that are important to them. And we’re finding new approaches to help individuals remain independent and at home as long as possible.

 

Specialized Services for Older Adults

Rehabilitation plays an important role in helping people manage illness or disability related to aging. It helps people manage chronic diseases like diabetes and osteoporosis or cope with the effects of Alzheimer’s and dementia.  Rehabilitation maintains or restores strength, mobility, thinking and memory – all of the things that help people to stay independent.

Older adults receive care in many of our services. However, we also have a variety of services that are specifically designed to meet their needs: At Toronto Rehab, care is individualized. Our interprofessional health team works with individuals and their families to set meaningful goals for their needs and situation. We focus on superior outcomes for each and every patient.
