At Toronto Rehab, we are committed to enhancing the quality of life of people who are approaching the end of their life.
Our Palliative Care Service provides compassionate, holistic care that allows individuals to live and die with as much comfort and dignity as possible.
Patient and Family-focused Care
We believe that to deliver excellent care, each person’s particular needs and realities must be taken into account. Our interprofessional health care team works with individuals and their families to develop an individualized care plan.
Patients admitted to our Palliative Care Unit have complex physical and medical needs and a life expectancy of three months or less. With that in mind, emphasis is placed on:

pain management, symptom control and physical comfort

emotional, psychological and spiritual support

supporting patient and family involvement, input and need for information

cultural sensitivity
Family members are welcome to visit whenever they wish and are supported in sharing their loved one’s journey as desired.
Home-like Environment Adds Comfort
The Palliative Care Unit provides a home-like environment to make individuals and families as comfortable as possible. Soft colours, warm fabrics and artwork create a soothing atmosphere.
The 10-bed unit has six private and two semi-private rooms, each with a bathroom. Each room has a sleeper chair that folds out into a bed so family members can stay overnight. There is a shower for families and a family lounge with a television, fridge and microwave.
A “quiet room” on the unit can be used for added privacy.
Interprofessional Approach Enhances Care
At Toronto Rehab, our patients benefit from the expertise of a wide range of skilled professionals who work together in an interprofessional team. Each team member contributes specialized knowledge to the care we provide. Patients and family members are integral members of the team.
