Long Term Care
Maximizing Life in Long-Term Care
As people age, illness, frailty or disability may make it impossible for them to live independently. Sometimes 24-hour nursing and personal care is required. Long-term care may be the best option.
Toronto Rehab opened its own long-term care facility in 2004, allowing us to provide exceptional 24-hour care for individuals in a residential setting.
Toronto Rehab—Maximizing Life
At Lakeside Long-Term Care Centre, we maximize life. Care is holistic and resident-centred. We help people maintain their mental and physical well-being so that they have the best possible quality of life.
Our nursing and personal care staff provides support for residents in all activities of daily living, as they need it. Our goal is to maximize function and help individuals live as independently and safely as possible. With that in mind, our care also includes physiotherapy and other therapeutic programs. Recreational programs provide individuals with opportunities to stay active and socialize.
Outstanding care is delivered by an interprofessional health care team that includes nurses, healthcare aides, activity and therapy staff, a social worker and dietary staff.
