Heart health is a lifelong commitment.

Toronto Rehab offers Heart Health for Life presented by Scotiabank to all graduates of the Cardiac Rehabilitation and Secondary Prevention Program.  

This graduate program provides tools and resources to help people maintain heart healthy lifestyles after completing their formal rehabilitation programs. It also helps graduates and their families access a broad network of support. The program provides:

 

  • Pulse Check Annual Fitness Assessment - All graduates are invited to return for free annual cardiopulmonary fitness testing to help them keep track of their heart health. This test will also provide an opportunity to safely adjust their exercise program. Download the Pulse Check referral form.

  • Take C.H.A.R.G.E. Alumni Education Series - Toronto Rehab, in partnership with the Cardiac Rehab Graduates Association (CRGA), presents monthly education for graduates and their families and supporters. Take C.H.A.R.G.E. helps participants stay up-to-date on the latest developments in exercise, nutrition and heart health. Sessions are presented by Toronto Rehab experts and guest speakers.   • Women with Heart Seminar Series for Women - Women with Heart is a free, quarterly education series open to the public. Experts are invited to provide the most-up-to-date information on women’s heart health. Women and men are welcome!

  • Staying on Track Drop-in Exercise Sessions - Our Staying on Track program provides drop-in exercise sessions at Toronto Rehab twice a week. A cardiac rehabilitation supervisor is there to answer questions and provide support. Learn more about the details and fees.

  • Heart Wise Exercise - The Heart Wise Exercise Program helps graduates transition from their formal rehabilitation program to fitness programs available in the community. The Heart Wise symbol identifies exercise programs that are safe and suitable for participants with heart disease. Classes take place at YMCAs in Toronto.   • Heart Health for Life Champions Graduate Volunteer Program - Our program graduates are our greatest champions. Heart Health for Life Champions invites graduates to stay involved with our program as volunteers. Volunteers stay motivated about their own heart health and inspire current program participants.

Community Resources

There are many resources available in the community. Program participants receive an up-to-date listing of resources such as:
  • Community resources - other cardiac programs, support groups, counselling and stress management programs

  • Diabetes resources

  • Fitness and community centres

  • Education resources

  • Walking resources - walking clubs, maps, mall walking programs
