Complex Medical Services

Caring for People with Complex Medical Needs

More and more people are living with chronic and complex health issues such as diabetes, kidney disease, heart disease and Alzheimer’s disease.

Approximately 90 per cent of people over age 65 have at least one chronic disease. Eighty per cent have at least two.

As we age, managing these diseases can be challenging—for individuals themselves, and for the medical community. When medical issues arise, they are often complex. A person’s health and ability to live independently can quickly deteriorate.

Toronto Rehab—Maximizing Life

Toronto Rehab has special expertise in providing care to individuals with complex medical needs.

We help people to recover their strength and mobility. We help them to manage their diseases or health issues to improve their overall well-being and restore their independence. We maximize life.

Our Hemodialysis Unit for seniors with advanced kidney disease is the first of its kind in a rehabilitation setting. Our unique approach to delivering hemodialysis is especially designed for older adults with multiple health issues. It helps them to regain the strength they need to participate in rehabilitation and regain their strength and independence.

We are also a leader in the care of pressure ulcers (bed sores.) Pressure ulcers are a significant problem for individuals who are immobile in bed for long periods of time due to illness or injury.

Individualized Care for Better Outcomes

At Toronto Rehab, our model of interprofessional care means that patients benefit from a diverse team of health care professionals with specialized skills and expertise.

We work with individuals and their families to develop an individualized treatment plan that meets their needs. We focus on superior outcomes for each and every patient.

Finding Solutions

We integrate the latest research into all the care we provide. Our researchers work side by side with the health care team so our patients can participate in research and benefit from discoveries—as we make them.

We’re also sharing our innovative interventions, clinical practices and technologies with rehabilitation providers across the country and around the world.
