Psychogeriatric Outreach

Toronto Rehab is a leader in rehabilitation for older adults who have complex cognitive and psychosocial needs.  We share this expertise with long-term care homes and retirement homes through our Psychogeriatric Outreach Service.

Individuals with Alzheimer’s disease or dementia may exhibit challenging behaviour such as:

• agitation
• wandering
• aggression
• resistance to care, and
• lack of awareness of their own needs

These behaviours can be difficult to manage in retirement or long-term care facilities.

Through our Outreach Service, a geriatric psychiatrist and an outreach clinician will do specialized assessments of residents and provide caregivers with recommendations for appropriate behavioural management strategies. When necessary, individuals can be admitted to our inpatient Geriatric Psychiatry Rehabilitation Service.
