Conference Rates
$2000.00 Individual Rate for Physicians
$1,500.00 Individual Rate for Nurse Practitioners
$   650.00 Individual Rate for Residents

Registration closes at 5:00 pm on Thursday January 12, 2012.
Payment by VISA, MasterCard or cheque must accompany the completed online registration form. All cheques must be made payable to the order of Toronto Rehabilitation Institute. Cheque payments must be received on or before Monday December 20, 2010. Registration deadlines and fees are subject to change.

Cancellation Policy
A full refund, minus a $75 administration fee, will be issued for cancellations received via facsimile (416-597-6202) or e-mail on or before Monday December 19, 2011. No refunds will be issued after this date. Refunds will not be granted to registrants who do not attend the course.
