Guidelines
- The consumer outreach fund has been developed to engage consumers from across Canada in attending this scientific conference.
- A travel grant will be provided to attend the conference only, starting Thursday night running to Saturday afternoon. Consumers applying for this support are expected to attend the full conference including a wrap up lunch Saturday and workshops that afternoon. Consumers unable to fulfill this commitment are not eligible for this grant.
- Consumers wishing to attend the pre-course or attend outside activities while in attendance at the conference are responsible for all costs related to these activities and will ensure that these activities do not limit their attendance at the conference.
- Preference will be given to those consumers who have not been previously supported by this fund however those who have been previously supported may apply and depending on the applications, may be considered.
- Consumers applying to this fund will be responsible to attend the full conference including a Thursday night consumer welcome dinner, meals provided at the conference and a wrap up lunch on Saturday. Individuals who are unable to obtain transportation home Saturday evening will have to book for an additional night at the hotel.
- Consumers attending the conference will be required to provide an article of one to two pages regarding the conference outcomes and provide information on how they will engage consumers in their home communities.
- Consumers will be expected to book their own travel, hotel and conference arrangements after receiving the travel grant.
- Attendant services will be provided at the conference. Regular attendant services required by the consumer will be supported by the recipient. Please note, newly adopted regulations allow attendants to fly at no charge.
- Travel grants will be given based on the geographic distribution of the applicants and the budget available.
- Applications will be confidential and available only to partners’ representatives involved in the review.
- Recipient will be asked to sign a consent form in which they agree to be interviewed and/or photographed by Toronto Rehab or any funding partners.
Application Process
The fund will be advertised by the Conference organizers to provide information to a wide audience. Application forms are available both on line and in hard copy. Applications for travel grants are due on August 2, 2010 by 5:00 PM and will be sent directly to the Toronto Rehabilitation Conference Services. All applications received will be reviewed by the partner panel. Successful applicants will be notified no later than August 20, 2010.
