**Students must present a photocopy of a valid student ID
***This rate is per OTN site and applies for participants who would like to view the plenary sessions on the Ontario Telemedicine Network.

Registration closes at 5 p.m. on May 8, 2012. Onsite registration will be from 07:45 – 08:45 am on May 11, 2012.

Payment by VISA, MasterCard or cheque must accompany the completed online registration form. All cheques must be made payable to the order of Toronto Rehabilitation Institute. Cheque payment must be received on or before April 20, 2012.

Registration deadlines and fees are subject to change.

Cancellation Policy

A full refund, minus a $75 administration fee, will be issued for cancellations received via e-mail on or before April 20, 2012. No refunds will be issued after this date. Refunds will not be granted to registrants who do not attend the conference.
