Our uniquely interprofessional approach ensures that we draw on the best evidence from every profession.
 
Our teams work together to identify patient needs. We review current and best practice and if there are gaps, we create strategies to address them.
 
We support staff in developing the skills they need to identify and implement best practice in all the care they provide.
 
And, most important of all—we ensure that any change is in the best interest of the patient.
 
Raising the bar
 
As a leader in rehabilitation research, we’re not only implementing best practice—we’re developing it. We’re creating new models of care and integrating research discoveries to constantly improve the care we provide.
 
Some examples of best practices developed at Toronto Rehab include:
 
·        Care for Hip Fracture Patients - Working with the Total Joint Network, we pioneered a new and innovative model of care for hip fracture patients that integrates best practice and begins rehabilitation much earlier. Patients spend less time in hospitals and more are able to return to independent living.
 
·        Cardiac Rehab – Our home-based cardiac rehab program is allowing us to extend the reach of our highly effective program to individuals living outside large centres or in underserved areas.
 
·        Falls Prevention – Successful rehabilitation requires patients to take risks and push their limits. Reducing falls and their resulting injuries is the focus of our organization-wide initiative, Stop Adverse Fall Events (SAFE). The initiative was awarded a leading practice by Accreditation Canada.
