Quality Improvement Plan
Measuring our performance, comparing our results to those achieved by other similar health care providers and monitoring our progress toward the achievement of our strategic goals are critical to continuous improvement and to the well-being of the people we serve. As a publicly funded institution, reporting our results to the people of Ontario is equally as important.
In June 2010, the Ontario Government passed the
Excellent Care for All Act. The legislation sets out a number of requirements for hospitals that include establishing quality committees, implementing patient and care provider satisfaction surveys, developing a declaration of values following public consultation, and establishing a patient relations process to address and improve the patient experience.
One of the ways that the Excellent Care for All Act is helping hospitals meet our community’s expectations regarding quality, patient safety and accountability is through the public reporting of Quality Improvement Plans. Toronto Rehab is now making available online its annual Quality Improvement Plan which will contain a number of indicators and targets for improvement that address improvements in Safety, Effectiveness, Access and Patient Centered Care.
Our commitment to Excellent Care for All
At Toronto Rehab, we use a variety of indicators to evaluate different aspects of our performance to get a balanced perspective of how we are doing.Our compliance with the Excellent Care for All Act builds on this existing long-standing commitment to quality and safety on which we build our strategic plan and annual operating plans. This commitment is focused on:
· Optimizing accessibility of rehabilitation services
· Employing the most effective practices
· Ensuring efficient delivery of care
· Ensuring safe practices and processes; and
· Meticulous monitoring and provision of continuous feedback
We do this through:
· The alignment of our annual priorities with our strategic directions and core values that are focused on advancing the quality of our health services and facilitating continuous improvement. The integration of our Strategic Plan and operating plans, along with our Hospital Annual Planning Submissions, Hospital Service Accountability Agreements and work on Accreditation Canada’s Required Organizational Practices, helps ensure the highest possible quality and accountability to our patients and our community.
· Participation in Accreditation Canada’s regular review process which evaluates health organizations against national standards of care delivery. Toronto Rehab was awarded full accreditation with no condition in 2010, and was recognized for several leading practices related to patient care quality and safety.
· The work of the Quality Committee of the Board of Directors, which is responsible for quality monitoring, analysis and ongoing improvement efforts. The members of our Board and the Quality Committee work closely with our community and with our team of health care professionals to determine areas where we are doing well and areas where we have room for improvement, with performance targets set on an ongoing basis.
· The work of the Patient Relations and Patient Safety teams. They lead or coordinate initiatives aimed at supporting our annual and strategic priorities as they relate to quality and safety throughout the entire organization.
· The use of a balanced scorecard which we use to regularly report on annual hospital performance internally and with the Board and aids in the production of our online annual reports. We report on indicators that relate to quality and safety and benchmarked against hospitals throughout the province.
· The use of patient and family satisfaction surveys as well as staff satisfaction surveys. We conduct surveys annually, compare our results with other hospitals and use this information to inform quality improvement efforts and annual planning activities.
· The ongoing production of an online Performance Report. This serves as an important mechanism for us to regularly share, in an open and transparent manner, our performance in a number of areas related to patient care, education, research, people and our relationships with, and accountability to, our stakeholders.
